Patient Care Operations Coordinator
This is a phone based role, onsite with our client in Newark, DE.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Essential Job Functions:
1. Management of home health patient schedules in collaboration with clinicians, aides and managers:
a. Ensures completed schedules a day, week or month in advance.
b. Continuous maintenance of schedule throughout the course of the day.
c. Communicate schedule changes to clinicians, aides and patients.
d. Ensure all clinicians and aides are scheduled adequately to achieve budgeted productivity.
e. Assures assignment of patients/services meets timely access to care requirements.
f. Coordinate and perform daily/weekly reports and reconciliation activities to ensure patient safety is maintained.
g. Maintain accurate database of employee profile information, availabilities, schedules, and patient demographics.
h. Process payroll reports and data entry.
i. Communicate/escalate any conflict in schedules to management.
2. Demonstrates ability to multitask; maintaining professionalism while dealing with competing priorities and deadlines.
3. Build professional relationships with patients to ensure excellent service delivery.
4. Other duties as assigned.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Scheduling and Home Health Care experience preferred
High School diploma (or GED equivalent), Associate degree preferred
Proficiency in Microsoft Outlook, Word, Excel and PowerPoint
Proficiency in using an electronic health record
Ability to work a flexible schedule, including weekends and holidays
Must be detail oriented
Must have good interpersonal skills
Must be able to multi-task and prioritize when needed
Must possess excellent communication skills
Must possess good coordination and analytical abilities
Must attend team and staff meetings as required
Must be able to work effectively as part of a team and independently
