Required Experience:  Ambulatory setting experience, injections, infusions, phlebotomy, Line care including care of IV and PICC lines – NOTE that worker will not place IV and PICC lines.

Required Certifications: Graduation from an accredited LPN program and pass the National Council Licensure Exam

Mon-Fri 8am-5pm

No on call, no weekend shifts

§  Administration of medication including but not limited to infusions, SQ/IM injection and oral

§  Line Care, including care of IV and PICC lines

§  Phlebotomy

§  Identifies patient needs and escalates to next appropriate resource as warranted

§  Communicates patient needs/requests to appropriate caregivers.

§  Reviews discharge instructions with patient.

§  Review of care plan with patient as well as patient’s care team

§  Schedules any necessary follow-up visits

§  Collaborates with other members of the care team

§  Provides scripted patient education

§  Receive delegated tasks from Physician/APC/RN

§  Participates in process improvement strategies

§  Participates in a relationship-based care approach to care delivery

§  Daily telephonic or in person contact with patients, physicians, and other members of the medical and front office staff as well as other departments located within the Medical Group.

§   interacts with outside medical offices and diagnostic centers to provide additional resources and support.
