Intake Coordinator (Home health experience preferred)

PRIMARY FUNCTION:

Under the direct supervision of the Administrative Supervisor, the Intake Coordinator is responsible for receipt and development of referrals from nurse liaison. Must ensure complete admission/financial information is obtained and entered correctly into computer system. This position is an administrative role within the HomeHealth office of the hospital. This role is similar to an Office Assistant. All interested candidates must have covid vaccine to be considered.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

● Receive and develop referrals from all Liaisons.

● Review all intakes for services requested working closely with RN/LPN.

● Maintain current system of tracking clients who have been admitted to the hospital and are pending hospital discharge.

● Assist in coordination of services to ensure a smooth, effective start of care.

● Respond timely to all correspondence received from internal and external sources.

● Input initial admission and financial information into the computer system.

● Verify physician’s license.

● Maintain our current referral tracking system to ensure all patients are seen timely.

● Make and distribute referral copies.

● Assist Admissions Coordinator as needed.
